Application

All activities regarding vertebrate animal use must be submitted with the Animal Care and Use Application. The form can be emailed to IACUC@missouristate.edu or Johnna Pedersen.  Please contact Johnna Pedersen at 417-836-3737 for more information, or questions.

Supplemental animal research & teaching forms

All information must be submitted on the appropriate form. Submissions on outdated forms will not be reviewed. See Application Instructions for how to submit the following forms.

Additional information

Prior to any animal activity, all covered personnel are required to complete Missouri State's Animal Care & Use Training as well as enroll in the Occupational Health & Safety Program (OHSP).
